Print out the Direct Withdrawal Form.
Please fill it out, sign it and return it along with a voided cheque.

Normally it takes about 3 to 6 weeks to process after we receive the form. You will have the choice of date, amount and length of time you wish to have withdrawals made.

A tax receipt is sent out in January for the total amount of contributions received for the previous taxation year. (Jan-Dec)

You may cancel the deposits at any time by contacting your bank or Child Haven.
